Pulmonary Nodules & Lung Cancer

Screen high-risk smokers yearly with low-dose CT, size incidental nodules against the 6- and 8-mm Fleischner thresholds, and recognize lung cancer’s inpatient complications.

Key numbers
Annual LDCT: age 50–80, ≥ 20 pack-years50–80 y · ≥ 20 pack-yr
Solid nodule < 6 mm: no follow-up< 6 mm: no follow-up
Nodule > 8 mm: PET-CT or biopsy> 8 mm: work up
01 · The ruleAnnual LDCT: age 50–80, ≥ 20 pack-years

USPSTF: annual low-dose CT for adults 50–80 with ≥ 20 pack-years who smoke now or quit within 15 years; stop after 15 smoke-free years or when health limits curative surgery.

02 · The numberSolid nodule < 6 mm: no follow-up

Fleischner 2017, incidental solid nodule: < 6 mm needs no routine follow-up if low risk (optional 12-month CT if high risk); 6–8 mm gets CT at 6–12 months, then consider CT at 18–24 months.

03 · The numberNodule > 8 mm: PET-CT or biopsy

A single solid nodule > 8 mm: consider CT at 3 months, PET-CT, or tissue sampling; PET-CT is unreliable for lesions smaller than 8 mm.

Presentation

What you'll see
Pancoast tumor: shoulder pain and Horner syndromeAn apical (Pancoast) tumor invades the brachial plexus — shoulder and arm pain, hand weakness and atrophy — and the sympathetic chain, causing Horner syndrome (ptosis, miosis, anhidrosis).
02

Diagnosis & monitoring

Work-up
LDCT cut lung cancer deaths by 20%In the National Lung Screening Trial, annual low-dose CT lowered lung cancer deaths by 20% relative to chest radiography.
Ground-glass nodule ≥ 6 mm: follow 5 yearsA pure ground-glass nodule ≥ 6 mm gets CT at 6–12 months, then every 2 years to 5 years; a persistent part-solid nodule whose solid part is ≥ 6 mm is highly suspicious.
03

Management

Orders
Tissue diagnosis with molecular testingLung cancer needs pathologic confirmation, ideally from a sample that also stages the disease, with biomarker testing in NSCLC for targetable driver mutations (EGFR, ALK and others).
Stage with PET-CTStage with PET-CT (or CT chest–abdomen–pelvis), biopsy suspicious mediastinal nodes, use MRI for apical (Pancoast) tumors, and check calcium and alkaline phosphatase for bone or paraneoplastic disease.
04

Red flags & don'ts

Avoid harm
Small cell: SIADH; squamous: hypercalcemia complicationClassic paraneoplastic pairs: small cell → SIADH (hyponatremia) and Lambert-Eaton syndrome; squamous cell → hypercalcemia from PTH-related protein; clubbing and hypertrophic osteoarthropathy mostly with NSCLC.
SVC syndrome: facial swelling, distended neck veins complicationSVC syndrome from tumor compression causes head fullness, facial and arm swelling, breathlessness lying flat, distended neck and chest veins and plethora; treat the cancer — chemotherapy for SCLC, radiation for NSCLC.
Malignant effusion: tap, then pleurodesis or catheter complicationA symptomatic malignant pleural effusion is drained by thoracentesis first; if it recurs, talc pleurodesis (> 90% effective) or an indwelling pleural catheter. Asymptomatic effusions need no treatment.
05

The rules

Know these
Fleischner rules don’t cover screening nodulesFleischner guidelines apply to incidental nodules in adults ≥ 35 — not to nodules found on lung cancer screening (managed by Lung-RADS), immunocompromised patients, or patients with a known primary cancer.
ACS 2023: years since quitting don’t matterThe 2023 American Cancer Society guideline screens ages 50–80 with ≥ 20 pack-years whether they smoke now or quit at any time — years since quitting is not a criterion.
06

USPSTF 2021 or ACS 2023 screening criteria?

Commonly confused
Age50–80
Smoking history≥ 20 pack-years
Former smokersOnly if quit within 15 years
TestAnnual low-dose CT
07

Commonly confused

Tell apart
Small cell: chemo-immunotherapy, rarely surgerySmall cell lung cancer (about 15%) is aggressive and almost always smoking-related; treat with platinum-based chemotherapy — concurrent radiation in limited stage, added immunotherapy in extensive stage. Surgery has little role.
08

Teach & follow up

Patient education
Shared decision-making and smoking cessationBefore starting screening, hold a shared decision-making discussion about benefits and harms; anyone who still smokes gets cessation counseling and is connected to treatment.

Reference
Show 6 reference facts
Local and regional spread causes hoarseness (recurrent laryngeal nerve), an elevated hemidiaphragm (phrenic nerve), dysphagia, pericardial effusion or tamponade, and postobstructive pneumonia.
Multiple solid nodules 6–8 mm or larger: CT at 3–6 months, then consider CT at 18–24 months, managing by the most suspicious nodule.
About 15–20% of lung cancers occur in people who never smoked or smoked minimally; household radon is the second leading cause in the US, and COPD and pulmonary fibrosis raise risk.
PET-CT is sensitive with an excellent negative predictive value but imperfect specificity; it is less useful for small, low-metabolic or low-cellularity lesions.
CT-guided transthoracic needle biopsy diagnoses cancer with about 90–95% accuracy but causes pneumothorax in 9–30% of patients.
Early palliative care in advanced lung cancer reduces end-of-life chemotherapy and may extend life.
Pearl

Think 6 and 8 for an incidental solid nodule: under 6 mm let it go, 6–8 mm rescan, over 8 mm work it up.
