Anticoagulant Reversal & Bleeding

Stop the anticoagulant and match the antidote: 4F-PCC plus IV vitamin K for warfarin, idarucizumab for dabigatran, 4F-PCC for factor Xa inhibitors, protamine for heparin.

Key numbers
Major bleed: stop anticoagulant, control the sourceHb drop ≥ 2 g/dL
Dabigatran: idarucizumab 5 g IV5 g IV
Factor Xa inhibitors: give 4-factor PCC4F-PCC 25–50 units/kg
Heparin: protamine 1 mg per 100 units1 mg per 100 units
01 · GiveWarfarin: 4F-PCC plus IV vitamin K

For major or life-threatening bleeding on warfarin, give 4-factor PCC (preferred over FFP) plus vitamin K 10 mg IV: PCC corrects the INR within minutes, vitamin K sustains the reversal once PCC wears off.

02 · GiveDabigatran: idarucizumab 5 g IV

Reverse dabigatran with idarucizumab 5 g IV (two 2.5-g vials), which binds dabigatran within minutes; if it is unavailable, give PCC or activated PCC.

03 · GiveFactor Xa inhibitors: give 4-factor PCC

For life-threatening bleeding on apixaban, rivaroxaban or edoxaban, give 4-factor PCC (25–50 units/kg). Andexanet alfa was withdrawn from the US market in December 2025 after excess thrombosis.

The rules

Know these
Major bleed: stop anticoagulant, control the source must knowA major bleed involves a critical site, causes hemodynamic instability, drops hemoglobin ≥ 2 g/dL or needs ≥ 2 units of red cells: stop the anticoagulant, control the source and resuscitate.
No reversal agent for nonmajor bleedingReversal or hemostatic agents are for life-threatening bleeding or major bleeding that persists despite initial care; most nonmajor bleeds need only a held dose and local measures, because reversal agents are prothrombotic.
Reassess restarting once bleeding is controlledOnce bleeding is controlled, stop anticoagulation if thrombotic risk is low (AF with CHA₂DS₂-VASc < 2–3, VTE provoked > 3 months ago); delay restart after a critical-organ or unexplained bleed; otherwise restart when safe.
02

Diagnosis & monitoring

Work-up
Dose 4F-PCC for warfarin by INR and weightLabeled 4F-PCC dosing for warfarin: INR 2 to < 4, 25 units/kg (max 2500); INR 4–6, 35 units/kg (max 3500); INR > 6, 50 units/kg (max 5000). Fixed doses are also used.

Why each step
Major bleed: stop anticoagulant, control the sourceA major bleed involves a critical site, causes hemodynamic instability, drops hemoglobin ≥ 2 g/dL or needs ≥ 2 units of red cells: stop the anticoagulant, control the source and resuscitate.
Warfarin: 4F-PCC plus IV vitamin KFor major or life-threatening bleeding on warfarin, give 4-factor PCC (preferred over FFP) plus vitamin K 10 mg IV: PCC corrects the INR within minutes, vitamin K sustains the reversal once PCC wears off.
Factor Xa inhibitors: give 4-factor PCCFor life-threatening bleeding on apixaban, rivaroxaban or edoxaban, give 4-factor PCC (25–50 units/kg). Andexanet alfa was withdrawn from the US market in December 2025 after excess thrombosis.
Dabigatran: idarucizumab 5 g IVReverse dabigatran with idarucizumab 5 g IV (two 2.5-g vials), which binds dabigatran within minutes; if it is unavailable, give PCC or activated PCC.
Heparin: protamine 1 mg per 100 unitsReverse unfractionated heparin with protamine 1 mg per 100 units of heparin still active (half-life 30–60 minutes, so count roughly the last 2–3 hours), given slowly IV over 10 minutes, no more than 50 mg.

Management

Orders
Enoxaparin: protamine reverses it only partiallyFor enoxaparin given within 8 hours, give protamine 1 mg per 1 mg enoxaparin (0.5 mg per mg if given 8–12 hours ago); protamine neutralizes only part of LMWH's anti-Xa effect.
IV vitamin K: infuse slowly, acts in hoursIn bleeding, give vitamin K IV, infused over about 30 minutes; it takes hours to restore factors, which is why PCC is given with it. Oral vitamin K is used when there is no bleeding.
High INR without bleeding: hold warfarinWith a supratherapeutic INR and no bleeding, hold 1–2 warfarin doses, recheck the INR and resume at a lower dose; give oral (not IV) vitamin K when the INR exceeds 10. No PCC.
Activated charcoal for a recent DOAC doseActivated charcoal is an option when the last DOAC dose was recent: within about 2 hours for dabigatran or edoxaban, 6 hours for apixaban, 8 hours for rivaroxaban.

Red flags & don'ts

Avoid harm
4F-PCC: thrombosis warning, contraindicated in HIT avoid4F-PCC carries a boxed warning for arterial and venous thromboembolism and is contraindicated in known HIT (it contains heparin) and in DIC; repeat dosing is not recommended.

4F-PCC or plasma for warfarin bleeding?

Commonly confused
RoleFirst line for major warfarin bleeding
ContentsFactors II, VII, IX, X plus proteins C and S
SpeedFaster INR correction and faster infusion

07

Commonly confused

Tell apart
Hemodialysis removes dabigatran, not Xa inhibitorsHemodialysis can remove dabigatran because it is not highly protein bound; it does not remove apixaban, rivaroxaban or edoxaban.

More detail

Reference
Show 6 reference facts
A normal thrombin time or aPTT (sensitive reagent) usually excludes clinically relevant dabigatran; normal anti-Xa activity excludes clinically relevant factor Xa inhibitor levels.
The FDA's December 18, 2025 safety communication judged andexanet's risks to outweigh its benefits; AstraZeneca withdrew it, effective December 22, 2025. ANNEXA-I: thrombosis 14.6% vs 6.9%, thrombosis-related death 2.5% vs 0.9%.
Three-factor PCC lacks factor VII; 4-factor PCC (factors II, VII, IX, X plus proteins C and S) is preferred because it reverses bleeding more effectively.
In trials, 4F-PCC brought the INR to ≤ 1.3 within 30 minutes in most patients; recheck the INR and clinical response after the dose.
If bleeding recurs with elevated coagulation tests, or a second urgent procedure is needed, an additional 5 g of idarucizumab may be considered.
Rapid protamine injection causes hypotension, bradycardia and anaphylactoid reactions; risk is higher after prior protamine exposure (eg, protamine-containing insulin), and doses above 50 mg can themselves anticoagulate.
