Caregiver Burden & Social Determinants of Health

A plan only works if the patient can afford it, get to it and has someone able to carry it out — screen social needs and caregiver strain, then adjust.

Key numbers
Zarit Burden Interview measures caregiver strainZBI-22: 0–88
Screen five core social needs5 core domains
Interpersonal safety screen: score 11 or moresafety score ≥ 11
01 · DoZarit Burden Interview measures caregiver strain

The Zarit Burden Interview measures perceived caregiver burden (health, emotions, social life, finances): 22 items scored 0 (never) to 4 (nearly always), total 0–88, higher = more burden; 12-item and 4-item versions exist.

02 · DoScreen five core social needs

The CMS Accountable Health Communities tool screens 5 core health-related social needs: housing instability, food insecurity, transportation problems, utility needs and interpersonal safety.

03 · DoAdjust the plan to social barriers

Use social findings in two ways: adjust clinical care to the barrier (no transportation → telehealth or fewer visits) and assist by connecting the patient to social care resources (National Academies awareness–adjustment–assistance framework).

01

What it is

Core idea
Caregiver burden can become burnout or abuse must knowCaregiver burden is the stress of caregiving, causing health problems, isolation, fatigue and frustration that can progress to burnout or elder abuse; it grows with the care recipient's disability and is especially heavy in dementia care.
02

Causes & risk factors

Why it happens
Cost and cognition drive nonadherenceMedications may fail because the patient cannot afford or obtain them or cannot manage complex regimens; ask about cost and access, and simplify the regimen before escalating therapy.
Isolation and no caregiver mean unmet needsSome older adults have no available, willing or capable caregiver and live with unmet needs and social isolation — a risk for self-neglect and, when care is shared, for abuse.
Caregiving carries financial and health costsCaregiving often brings lost wages, reduced work hours and financial strain, plus caregiver depression and stress-related illness; spousal caregiving couples are disproportionately poor.
03

Management

Orders
Connect every caregiver to social workRefer caregivers of older adults to social work to assess support needs and eligibility for training, respite, financial support and services; effective help includes care coordination, education, counseling, respite and practical assistance.
Confirm the caregiver can do the jobWhen the patient needs help with ADLs, determine whether the current caregiver has the emotional, financial and intellectual ability for the task before discharge; if not, identify a new caregiver or services.
Use health literacy universal precautionsAssume any patient or caregiver may struggle with health information: simplify communication and confirm understanding with everyone, and make care easier to navigate (teaching methods in the Patient Education sheet).
Interpersonal safety screen: score 11 or moreAHC safety items ask how often anyone physically hurts, insults, threatens or screams at the patient (1–5 each); a total of 11 or more suggests the patient may not be safe — evaluate for abuse.
Offer respite, adult day care and home healthSupport overwhelmed caregivers with adult day care, respite programs and home health care, found in the US through the Eldercare Locator or Area Agencies on Aging; point out burnout effects to caregivers.

Family caregivers provide about 89% of help at home for older adults, averaging about 22 hours a week; about 19% give more than 41 hours.
The 12-item ZBI correlates 0.92–0.97 with the full 22-item version and the 4-item screen 0.83–0.93.
The AHC food item asks whether, in the past 12 months, the patient worried food would run out before there was money to buy more; optional domains add financial strain, employment, substance use and mental health.
The FY 2026 IPPS final rule removed the Screening for Social Drivers of Health and Screen Positive Rate measures from the Hospital IQR program (costs outweighed benefit); hospitals may still screen as clinically appropriate.
Families of ICU survivors are at risk of anxiety, depression and PTSD (PICS-F); caregiving more than 100 hours a month and lack of support raise that risk (see Post-Intensive Care Syndrome).

Ask, adjust, assist: find the social barrier, change the plan around it, and connect the patient to help.
