Cholecystitis & Cholelithiasis

Cholelithiasis is gallstones in the gallbladder (often asymptomatic); cholecystitis is inflammation of the gallbladder wall, most often from a stone obstructing the cystic duct. Classic risk profile is the '5 Fs', though men and younger clients can be affected.

Vomiting
Low-grade fever
Other findings
Jaundice

Diagnostic

first-line; wall thickening, pericholecystic fluid, stones

Monitor

Elevated WBC
infection/inflammation
Elevated alkaline phosphatase
rises with CBD obstruction
Elevated direct bilirubin
rises with CBD obstruction
Elevated amylase / lipase
suggests gallstone pancreatitis
Keep NPO
rest the gallbladder pre-op
IV fluids
Antiemetics
Analgesics
pain control
Keep T-tube bag below insertion site
open chole; drains bile by gravity
Monitor T-tube bile color/volume
dark green, lightening over days
Never clamp T-tube without orderHold
Laparoscopic cholecystectomy Hallmark
definitive treatment
ERCP biliary decompression
for common bile duct stone / cholangitis
Start low-fat diet post-op
small, frequent meals
Reintroduce fat over 4-6 weeks
liver adapts to continuous bile flow; not permanent restriction
Report clay-colored stools
NOT expected; signals bile obstruction
Expect referred shoulder pain post-lap
residual CO2 insufflation; ambulate, usually resolves
Report Nowescalate immediately
Charcot's triad
RUQ pain + fever + jaundice = ascending cholangitis
Reynolds' pentad
Charcot's triad + hypotension + confusion = septic cholangitis
Hypotension
sign of sepsis
New-onset confusion
altered mental status with sepsis
Persistent clay-colored stools
complete biliary obstruction
